Requirements
Grant recipients should address the issues listed below when submitting
interim, annual and final progress reports to the Hoffberger Foundation.
Annual and final progress reports are required for all multi-year grants.
Deadlines for these reports are indicated in the grant agreement letter that
accompanies the first installment of the Hoffberger Foundation’s grant
commitment. The Hoffberger Foundation will request interim reports on an
as-needed and case-by-case basis. In some cases, additional information
will be requested by the Hoffberger Foundation. This, too, will be
requested on an as-needed and case-by-case basis.
Annual and final progress reports should follow the format below and should
not exceed five pages in length. If this is an annual report, your
responses should be relevant to the reporting period (e.g., what was
accomplished during the reporting period).
Progress reports should be submitted by regular mail, not by e-mail.

- List and briefly describe the overall project objectives and activities
you plan to accomplish with this grant.
- Describe and explain the reason for any changes to the objectives as
stated in the original proposal.
- Describe the activities and people involved.
- Describe the target population addressed by the grant.
- List and describe the outcomes you plan to achieve with this grant.
- Describe the measurement tools, strategies and/or approaches used to
determine the extent to which the project outcomes were achieved (attach any
relevant material).
- If any or all of these outcomes were not achieved, explain why.
- Explain any obstacles you encountered and changes in plans if different
from those stated in your original grant proposal.
- Describe any organizational changes that occurred during the year, such as
staff changes or growth in the number of people receiving services.
- Fundraising:
- Describe your fundraising goals.
- What was actually raised (identify sources)?
- Is there a difference between the goals and what was raised?
- Describe the highlights of implementing this project.
- Describe the barriers encountered while implementing this project.
- Describe the lessons that would be helpful to your organization or others
when considering implementing a similar project.
- Describe the extent to which this project will be continued, expanded or
replicated.
- Describe how you will use these results to inform decisions for future
improvements.
- Please attach copies of any publications that describe or are a result of
this project, including newspaper articles.
